A Partnership Built for Better Care
Care-Connect was formed in 2022 as a partnership between Centric Health and Irish Life Health. This innovative partnership allows for the provision of remote clinical support to address chronic disease management and reduce the need for hospital presentations.
Founded in 2003, Centric Health delivers patient‑centred primary care with a strong focus on preventative healthcare, chronic disease management and evidence-based care. Centric Health operates a network of over 80 GP practices across Ireland as well as practices within Germany and the Netherlands.
Irish Life Health was created in 2016 when two leading insurers came together to promote healthier lives. Today, Irish Life Health supports over 500,000 members offering the broadest range of benefits including ExpressCare Clinic, Digital Doctor, Healthy Minds, Back-Up, Minor Injury Clinics, as well as numerous wellbeing resources.

Our Patient Team
Care‑Connect is led and driven by a forward‑thinking, patient‑centred, multidisciplinary team dedicated to creating smarter, more compassionate ways of delivering care.
Across Ireland our specialist nurses, care co-ordinators and pharmacist provide remote clinical oversight and guidance to patients living with chronic diseases, leveraging digital technology and a CE approved application called Luscii Vitals.
An external expert advisory team in cardiology, respiratory medicine, sleep medicine and general practice help guide all our programmes.
