Digi-physical, Integrated, Hybrid or Blended Care describes the combination of digital patient monitoring and nurse-led clinical advice offered by Care-Connect .

Who are Care Connect?

Care-Connect was established as a partnership between Centric Health and Irish Life Health to provide digital, remote support to Irish Life Health members for heart and respiratory chronic diseases to lessen disease exacerbations and reduce the need for hospital presentations. Care-Connect provides oversight and guidance through specialist nurses, pharmacy, admin and a medical advisory support team in combination with a medically approved application.

What programmes do Care-Connect offer?

Care-Connect offers a chronic disease programme for Irish Life Health members with Heart Failure and respiratory conditions including COPD, Chronic Bronchitis, Emphysema or Bronchiectasis. Care-Connect also provides a home Sleep Apnoea Diagnostic Programme for patients with suspected sleep disordered breathing. Please refer to Irish Life Health to confirm your cover for this benefit.

Who can access Care-Connect programmes?

Care-Connect programmes are available to Irish Life Health members, aged 18+ , on all hospital plans who have been diagnosed with one of the conditions supported by our programmes. If you are unsure of your diagnosis you can discuss this with your GP or consultant.

How much do Care-Connect programmes cost?

Cover for the programme may be available on your health insurance plan. Please refer to Irish Life Health to confirm what is covered under this benefit. Terms and conditions apply. See your Table of Cover and Membership Handbook for details.

Do I need any equipment?

Care-Connect will provide you with all the medical equipment you need to participate in any of Care-Connect’s programmes. You will need access to a smart device such as a smart phone or tablet in order to participate in a care programme.

Is the medical App difficult to use?

No, the app provides step by step instructions and is intuitive to use. The Care-Connect admin team and your nurse are always available should you have any connectivity issues or queries. A family member or carer may also be available to help you.

Will my GP know what's happening?

Keeping your GP in the loop is an important part of your care plan. With your permission, we'll keep them updated on any changes or concerns with your overall health and clinical condition.

How is my data kept safe?

Data security is of upmost importance to Care-Connect. Care-Connect is the controller of all data collected during your care and has strict data protection processes and procedures in place.
