Care-Connect's virtual programmes integrate the abilities of technology and knowledge of real-live clinicians to support patients through their healthcare journey.

The number of people with chronic disease in Ireland is continuously increasing. With this increase in chronic disease, the healthcare system has come under pressure due to the growing demand, which means resources are constrained. As a result, people suffering long-term conditions often have limited access to the care they need, when they need it.

Care-Connect provides digitally enabled clinical support for our patients. Our programmes allow patients monitor their symptoms at home while we provide clinical advice and guidance when our patients show signs of deterioration, have questions or need reassurance.

Using the Luscii app, patients can record their clinical data while the Care-Connect team keep a close eye on changes in their measurements or signs of deterioration. 
The Care-Connect team can support patients to improve their symptoms by reacting at the first sign of trouble. Care-Connect has an experienced team of care coordinators and nurses to support you.
We can provide you with all you need to participate in the care pathway – including the appropriate equipment.

How do our patients benefit?

By carrying out regular home checks, patients are able to note and report any changes in symptoms at a very early stage. Early medical intervention can prevent further deterioration and hospitalisation. We aim to equip our patients with the knowledge required to improve their health habits and to be confident in recognising symptoms and managing their conditions at home. We expect to improve our patients' health outcomes so that they can have a better quality of life in their own community and home setting.

Our team will contact you to explain the programme and assess your eligibility for the programme.

When you have been successfully enrolled in the appropriate programme, we will ask you to submit your measurements via the app on designated days.

We will help you to download the Luscii app and provide all the necessary equipment. All you need is a smart phone or a tablet.

Highly qualified nurses review your results and symptoms on a regular basis.

If we recognise any signs of deterioration in your condition, one of our nurses will contact you to advise on further steps.

If you need additional assistance or advice, our team is there for you.

Am I eligible?

Please refer to Irish Life Health to confirm your cover for this benefit.

We currently run the following programmes:

  • Heart Care Programme- for those with a confirmed diagnosis of Heart Failure 
  • Respiratory Care Programme- for those with COPD, Chronic Bronchitis, Emphysema or Bronchiectasis
  • Sleep Apnoea Programme- for those with suspected sleep disordered breathing

You can participate from anywhere in the Republic of Ireland. You must be 18 or over and have access to a smartphone or a different smart device.
