Heart Care Programme

An innovative approach to improve the management of heart failure at home

Are you living with heart failure?

Heart Failure is a complex disease where the heart muscle isn't pumping blood as well as it should.

Cause

Although the exact cause may not always be clear, genetic predisposition, age or certain infections can be contributing factors and heart failure can be compounded by diet, lifestyle and medication non-compliance.

Symptoms

When the heart isn’t pumping blood as efficiently as it should, blood often backs up and fluid can build up in the lungs resulting in symptoms that include shortness of breath, fatigue, and swelling of the legs, feet and ankles restricting normal activity.

Treatment

While heart failure is not considered curable, it is managed with the right combination of medications, lifestyle changes, and ongoing monitoring. Early intervention and consistent treatment can slow the progression of the disease, reduce hospitalisations, prevent complications and improve quality of life.
